Management of Radial Tears During Capsulorhexis
نویسنده
چکیده
Radial tears during continuous curvilinear capsulorrhexis (CCC) are one of the most unwanted events that a surgeon may experience during cataract surgery. There are many factors that may play a role in inducing radial tears during CCC, such as a shallow anterior chamber, weak zonules as seen in pseudoexfoliative syndrome (PEX), high positive vitreous pressure, intumescent and hypermature cataracts, pediatric cataracts and a surgeon with minimal experience performing CCC. I describe a technique in which by aspirating the anterior epinuclear lens materials prior to phacoemulsification (PE), we can produce some free space between the nucleus and lens capsule and then can rotate the nucleus without complete hydrodissection and safely proceed with PE after cortex aspiration by engaging the nucleus between the phaco tip and a second instrument with the help of high vacuum and by cracking the nucleus without sculpting or chopping in order to minimize the stress on the zonules and prevent extension of radial tears during CCC.

Capsulorhexis rescue after peripheral radial tear-out: quick-pull technique.
UNLABELLED We describe a technique to rescue the continuous curvilinear capsulorhexis (CCC) in cases in which complete radial tears make it impossible to use normal traction forceps. A circumferential path and rapid movement are applied in the plane of the anterior capsule in the direction of the center pupil. This technique was used in 50 cases. In 47, the CCC could be completed; in 3, it coul...
متن کاملSpontaneous idiopathic true exfoliation of the anterior lens capsule during capsulorhexis.
We report a unique case of spontaneous per-operative idiopathic development of true exfoliation of the anterior lens capsule during capsulorhexis. A 45 year old female who underwent phacoemulsification for a cataract in the left eye, developed a partial split (the double ring sign) and eventual complete separation of the superficial lamina of the lens capsule during capsulorhexis. Radial tears ...
متن کاملDouble-ring and double-layer sign of the anterior lens capsule during cataract surgery.
BACKGROUND Splitting of the lens capsule with a double-ring contour during continuous curvilinear capsulorrhexis has not often been reported. CASE An 86-year-old woman underwent phacoemulsification cataract extraction with intraocular lens implantation in the right eye. Preoperatively, the anterior lens capsule appeared normal. OBSERVATIONS A double-ring sign was observed during capsulorrhe...

PURPOSE To compare the intraoperative safety of two techniques of capsulorhexis for intumescent white cataracts: traditional one-stage continuous curvilinear capsulorhexis and two-stage continuous curvilinear capsulorhexis. METHODS This prospective comparative randomized study included two groups: the 1-CCC group (11 patients) received traditional one-stage continuous curvilinear capsulorhexi...
متن کاملA novel technique of rescuing capsulorhexis radial tear-out using a cystotome.
UNLABELLED PART 1: PURPOSE To demonstrate a capsulorhexis radial tear out rescue technique using a cystotome on a virtual reality cataract surgery simulator and in a human eye. PART 2: METHOD Steps: When a capsulorhexis begins to veer radially towards the periphery beyond the pupillary margin the following steps should be applied without delay. 2.1) Stop further capsulorhexis manoeuvre and ...
